WEST CHESTER EAST CHEELEADING FORMS:

 

ADDITIONAL REQUIRED FORMS / ITEMS:

PIAA PHYSICAL FORM AND INFORMED CONSENT-

(ALL CHEERLEADERS MUST HAVE THESE FORMS COMPLETED AND TURNED INTO THE ATHLETIC OFFICE PRIOR TO CAMP.  ***MAKE SURE TO KEEP A COPY FOR YOURSELF)

The PIAA will only accept the PIAA Comprehensive Initial Pre-Participation Physical Evaluation form (CIPPE). This form can be downloaded and printed from the East webpage or can be picked up in the Athletic Office. These comprehensive physical forms will now be valid for 1 school year. They will not carry over into another school year. All CIPPE physicals will be good from June 1st to May 31st,  of the school year regardless of when the physical is done during the school year.

Again, all appointments for physical examinations must be completed on or after June 1st for the school year to follow that June. For more information about the CIPPE form, you can visit the PIAA website at www.piaa.org. If you would like information as to were one can get a PIAA physical for a reduced cost please contact the WC East athletic office at 484-266-3925.

West Chester Area School District Activity Fee

 

This fee must be paid directly to the West Chester Area School District. West Chester East nor cheerleading has anything to do with this payment.

 

The activity fee for participation in extra-curricular activities remains unchanged for the 2014-2015 school year. The fees for participation are as follows:   High School: $75/ Middle School: $50/ Elementary School: $25/ The maximum per family is capped at $200

 

Our district is committed to ensuring that no student will be denied extracurricular activity participation because of a concern regarding cost, therefore we also provide information regarding financial help. In order to help parents with a convenient, fair and seamless payment process, the district is again using PayForIt, an on line method of making a payment.

 

Directions for using this system can be found on individual school websites and on your school’s athletic homepage.  Please do not make payment until your son/daughter has been rostered into a funded activity.  

 

The following payment deadlines have been established: 2 weeks prior to the start date of the activity/sport.
